Employment Facilitator/QIDP/Case Manager - Swansea

The Employment Facilitator/QIDP (Qualified Intellectual Disability Professional) is responsible for Case Management for clients with disabilities and possibly the supervision of the employees who assist the vocational work and integrated goals.
The Employment Facilitator/QIDP establishes and implements client personal behavior and vocational goals with clients and related stakeholder input, provides instruction and feedback to clients for continuum of care and continuous improvement, evaluates the documents progress and ensures compliance with regulatory standards.
The Employment Facilitator/QIDP could assist the Director with interviewing, training, assigning work, evaluate and recommend discipline for employees.
Pay Rate:

Swansea Skills Training Center Job Duties Provide case management services for clients.
Assist the clients with vocational, personal, social, guardianship or advocacy needs:
Coordinate additional assistance from staff, client family members or guardians, residential provider and other service providers.
Implement client programming, evaluate progress and maintain supporting documentation.
Establish annual goals and objectives with clients and applicable family/guardians/residential provider/state counselor.
Write new plans and update existing plans.
Monitor progress and document behaviors, incidents and service activity.
Review staff's documentation and file all plans, reports and forms in case files per procedures.
Supervise the provision of work or activities on site or in the community.
Assign appropriate work or activities for clients and ensure that proper training, supervision and transportation, if needed, are provided.
Assist with reviewing tally sheets and the tracking system.
Supervise assigned staff.
Train and evaluate assigned Skills Trainers.
Communicate regularly with staff on clients' updates, work and other changes.
Make recommendations for improvement, discipline and assist with interviewing as needed.
Assist clients with activities of daily living (ADL) and production work.
Supplement staff efforts, or fill in for absent staff as needed, by helping clients with toileting, break and lunch time activity, readying for bus runs or supervising production work.
Transport clients as needed for community outings, off-site contract work, regular work commuting or other appointments using a company van or personal insured vehicle.
